One thing I have found helpful is a tighter t-shirt underneath to help the outer shirt slide better and deform less. If the sleeve cuffs hit your wrist at the normal spot, the buttons aren’t bursting, and the sleeve holes aren’t digging into your armpits, I’d say that’s a proper fit for a slim fit shirt.

No one else has addressed your neck question so I will say don’t change your neck size. It should be the smallest size that’s comfortable lots of people wear big necks nowadays anyway so it’s not a huge deal to go.

Instead of changing the neck, don’t get a slim fit shirt. Get the next fit up. If you can’t tell, take a cloth measuring tape with you.
